I realise that participation in dance classes and activities could involve some possible personal injury.
Despite precautions, accidents and injuries may occur. By signing this release form, I (the dancer) assume all risks related to the use of any and all spaces used by Cherry Steppers.
I agree to release and hold harmless Claire Cherry and Cherry Steppers from any cause of action, claims, or demands now and in the future. I will not hold Claire Cherry and Cherry Steppers liable for any personal injury or damage, which may occur on the premises before, during or after classes.
And I represent and warrant that I am physically fit and I have no medical condition that would prevent my participation in class.
